Observation

Regarding an unexplained bump on a child’s head noticed at the end of the day on 11/29/2022, the staff present could not explain how the injury occurred and did not document the injury. Further, the staff reported vacuuming the classroom that afternoon during a time when the classroom was at or near capacity and both staff were needed to meet required staff-child ratios and supervision. It was determined this allegation of lack of supervision regarding the incident of the unexplained bump on a child's head is ‘valid’. WAC 110-300-0345(3)(a)(b)(c)(d)(e)
